Palmetto Care Connections is a non-profit network that assists health care providers in connecting rural and underserved South Carolinians to quality services through broadband, technology and telehealth programs.
PCC is looking to enhance their team with a Telehealth Coordinator to play a key role in the coordination and communication of telehealth care in the community setting connecting health care providers directly to patients and/or community hubs.
This position provides customer service relations management and performance consulting services to health care providers and/or their patients to ensure proper clinical workflow.
This position will directly with health care providers, schools, community centers, business and industries and even grassroots organizations to increase access to quality services.
Essential Duties:
Is responsible for the successful implementation and ongoing management of Telehealth care in conjunction with standards, protocols, guidelines, policies, and procedures Work with schools, pharmacies, churches and/or other providers to set up telehealth programs Assist as a Tele-Presenter when needed Available as a resource to the nursing staff and other departments to provide education and training on the use of telehealth equipment and services Assists in orientation and development of programs within the community Keeps staff informed by interpreting and communicating telehealth rules, regulations, policies, and procedures.
Assist remote providers with workflow for telehealth services Assist in configuration/deployment/implementation of Remote Patient Monitoring services that PCC administers Participate in external training sessions.
Meet regularly with operational and telehealth team members to improve and streamline the process including with the marketing team to create and share marketing materials and updates.
Flexible schedule to meet department needs.
Some evening, weekends and extended hours may be required.
Administrative Assistance - Symposium, Summit, Annual Report, and as needed.

Education &
Experience:
Licensed Practical Nurse or Medical Office Assistant 2-4 years in healthcare setting - preferred Must have understanding of physical assessment & basic medical terminology Proficient with computers Ability to work independently A positive attitude Excellent communication skills Ability to work independently Position requires some day travel so applicant must have a car and a valid SC driver's license and limited overnight travel for conferences Job Type:
